17CE107 - Improving Access to Care Through Outreach
Purpose Statement
The purpose of this activity is to explore the importance of access to care and evaluate common barriers to care faced by patients. Two detailed case studies will enable the learner to apply information gained through this activity.
Educational Objectives
- List four potential barriers to care in vulnerable populations
- Define outreach as it relates to health care delivery
- Differentiate three telemedicine delivery methods
- Discuss considerations involved in planning and financing an outreach clinic
- Summarize the current consensus on the cost-effectiveness of outreach efforts
- Identify current barriers to care, appropriate interventions, and associated outcomes related to a review of detailed patient scenarios
Target Audience
This activity is intended for staff employed within the U.S Hemophilia Treatment Center Network, or USHTCN, to include nurses, social workers, physicians, physical therapists, administrators, and other professionals who wish to understand outreach as it relates to care in vulnerable populations. The activity may also be of interest to hospital or institutional staff responsible for planning and managing outreach programs.
Topics/Content
This module describes how the approaches to using outreach clinics may help overcome challenges to the delivery of specialty medical care to individuals with bleeding disorders.
